Declaration of decontamination
Whenever an instrument is returned for service it is essential that a completed
Declaration of Decontamination be completed for each and every item within the return
shipment.
It must always be attached to the OUTSIDE of the packaging, together with the shipping
documents, and be freely accessible. All shipped instruments, where the decontamination
statement is missing, will be kept unopened in our quarantine store until the declaration
has been forwarded.
